You start Flay (E) and put a point into it at level 4 to be able to create a bit of lane pressure after that you max out "Death Sentence" (Q) for the cooldown. If you flay someone backwards he is unable to dodge and can be grabbed without any risk

Prefferance and Match up

Usualy you wanna go with Exhaust since its super usefull in lane aswell as lategame. Always keep in mind: it doesnt only slow attack speed and movement speed but also reduces damage output by 40% and reduces Armor & MR by 10. Sometimes you know "I'll win my lane if i take ignite and make my adc snowball" If you know that you either have someone else in your team that runs exhaust (Like a yasuo, karthus or someone like that) you might aswell take ignite if the matchup allows it.

Alternatives

I usualy use a Bruiser-Type runepage for my Thresh as shown here but you might aswell take Movement speed quints and screw the CDR Glyphs & go for more MR glyphs, i preffer it this way though. Like this you can pressure the enemy team realy wenn at lv 1-4 while your adc farms. Since you start E and put a second point in it at lv 4, everytime you get your passive up u'll smack the adc for quite a chunk of hp.

Well, masteries.

Since the website isnt Updated yet i can't realy show it here. Heres a Screenshot of mine. http://www11.pic-upload.de/31.05.15/keyqc8jz9r1v.jpg As you notice i go for 5 Points in offensive because i like the CDR and when i have 4 in it i might aswell take the increased damage for allies and myself. I got 10 points in the Defensive Tree (9 cuz i want them and 1 because i dont know where to put it, other than into Enchanted Armor) You realy wanna take Recovery for the Health regen, block and Unyeilding for obvious reasons (Damage reduction) and the 4 points in Veterans Scars and Juggernaut are simply for Lane Survivability and lategame tankynes. 15 Points in the Utility Tree, to keep it short; Cookies for better sustain, gold masteries for, well, gold, a bit mobility increased warding range, reduced summoner spell cooldowns. Because as we know: Exhaust OP might aswell reduce its cooldown.
